Three things remove fluoride from water: reverse osmosis, activated alumina, and bone char. Standard carbon filters do not — and that includes most pitchers and faucet-mount filters, which is the thing people get wrong most often. This page answers the mechanical question and nothing else: what takes fluoride out, what does not, and where each one goes.
Most household filtration is carbon. Carbon works by adsorption — chlorine and organic compounds stick to the surface of the media as the water passes through. It is genuinely good at that, which is why a carbon filter makes town water taste better.
Fluoride is not one of the things it holds onto. It passes through the carbon and comes out the other side. So a pitcher, a faucet-mount cartridge, or a whole-house carbon filter can be doing exactly what it was built to do and still not touch fluoride at all.

Household pressure pushes water against a semi-permeable membrane; water passes through and most dissolved solids, fluoride among them, are rejected to the drain. This is the standard residential route, and it is the one we install. It goes under the kitchen sink, with its own dedicated drinking faucet on the countertop and a small storage tank in the cabinet. Its standard is NSF/ANSI 58.
A media bed that adsorbs fluoride out of the water as it passes through — a different chemistry than carbon, aimed at a different target. Activated alumina is a media rather than a finished system, so where it sits depends on the unit built around it: it turns up both as cartridges serving a single tap and as tank-style vessels plumbed into a line.
A carbon made from bone, which adsorbs fluoride in a way ordinary activated carbon does not. Like activated alumina it is a media, not a system, and it is most commonly built into cartridge-style filters serving one tap.
Of the three, reverse osmosis is the one we plumb in. Activated alumina and bone char are real technologies and worth knowing about — we are naming them because the honest answer to what removes fluoride is three things, not one.
Here is the detail that decides whether a system actually does what you bought it for. Under NSF/ANSI 58, fluoride is an optional claim. The standard covers complete reverse osmosis systems, and its contaminant claims are listed individually rather than bundled. A unit can be fully and legitimately certified under 58 without fluoride being one of the things it was certified for.
Which is the general rule stated in the specific case: certification is contaminant-specific. The standard number on the box tells you nothing on its own. Fluoride has to appear by name on that particular product listing. If the answer to whether it removes fluoride is that it is NSF certified, the question has not been answered — that is the moment to ask to see the listing.
Worth confirming before spending anything, because the answer is free and it is specific to your address.
On public supply, some Massachusetts public water systems add fluoride and some do not. Your town water department knows which, and so does the annual water quality report your public water system publishes for its customers. That is one phone call or one document.
On a private well, nothing is added to your water — there is no system upstream doing anything to it. Naturally occurring fluoride can be present in groundwater, though, and there is no way to tell by taste or by looking. A test at a certified lab is the only thing that answers it. Our own limits, stated plainly: we do not test for fluoride. We check hardness, iron, pH, and chlorine on-site, and we can collect a sample and send it out through our supplier to a testing company for a full water-chemistry panel with recommendations. That panel is water chemistry — it is not a fluoride test.

We do not install whole-house reverse osmosis. An RO membrane makes water slowly and sends part of it to drain, which suits a drinking faucet and does not suit the volume and speed a whole house asks of its plumbing.
